Licensing Inspector Scott conducted an unannounced self-report inspection in response to allegations received by the licensing office on 12/18/2023, in the areas of abuse & neglect.
The investigation of the self-report was initiated on 12/19/2023 and findings were reviewed on 12/22/2023. As the information gathered during the investigation did support the allegations, the complaint is determined to be ?VALID.?

Standard #: 8VAC20-780-410-1 Description: Based on staff interviews, the center failed to ensure that actions towards children that are forbidden by the standards (physical punishment, striking a child, roughly handling, or shaking a child) are not used at the center.
Evidence:
1.Staff 2 stated that she observed looking through the observation window of the classroom door, and witnessed Staff 1 standing over the child back. Staff 1 then pushed the child?s head into the carpet. Staff 1 then grabbed the child by both arms lifting the child to their face and scream ? DO NOT SPIT?.
2 Staff 2 stated they went into the class to talk to the staff member about what they witnessed. Staff 2 stated Staff 1 stated ?your right, I was frustrated, and he was spitting?.Plan of Correction: Staff member was terminated.
