Comments:
An announced initial inspection was conducted on July 24, 2023, to determine the centers compliance with licensing standards. The inspection was initiated at approximately 9:30 am and concluded at 10:50 am. The director participated in the inspection. The center plans to provide care to children ages five years, through twelve years eleven months and has requested a capacity of 75 children. The facility will operate in the All-Saints Catholic School with care being provided in the cafeteria, gym, and 1 classroom. Children will have access to the playground equipment at the side of the school. Transportation will not be provided. The facility is equipped with age-appropriate equipment and supplies. An afternoon snack will be provided to the children by the center. Medications will not be administered at this time. Standards related to supervision and program could not be determined as the facility is not yet fully operational. There were no children enrolled at the time of the inspection. The records of three staff members were reviewed. The first aid kit and non-medical emergency supplies. A conditional (6 month) license will be recommended. An unannounced follow-up inspection will be conducted within 60 days to determine compliance with standards not determined at the initial inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on review of staff records, the center did not ensure that all staff files had documentation of a completed Tuberculosis (TB) test/screening within the last 30 calendar days of the date of employment.
Evidence:
1.The record for staff 3 (DOH 7/1/2023) did not contain documentation of a completed tuberculosis (TB) test/screening.
2. The director stated that a tuberculosis (TB) test/screening was completed for staff 3, but the test results were not documented on placed in the file.
