Comments:
The Licensing Inspector conducted a joint investigation with a local agency in response to a complaint that was received by the licensing office on December 26, 2019. The investigation regarding slamming a child down onto a cot in the Three Year Old classroom was initiated on January 21, 2020 from 9:12am to 11:20am.

Interviews were conducted with staff and children and documents were reviewed.

The information gathered during this investigation does support the allegations, so the complaint is determined to be valid. See the Violation Notice for findings.

Description: Based on interviews and documentation, a staff member used forbidden actions of striking and rough handling children in care.
Evidence:
1. On 11/01/2019, Parent of Child #1 sent an email to Staff #3 reporting that Child #1 stated that ?Staff #1 threw {Child #2} down on her bed when she wasn?t listening at nap time.? Child #1 told her parent that ?Staff #1 had done it to her before too when she wasn?t listening at nap time.?
2. When interviewed, Child #1 (3 years old) stated "{Staff #1} hit { Child #2} because she was on {Staff #1's} lap." Child #1 stated {Staff #1} pushed {Child #2's} head down and {Child #2} (3 years) cried. Child #1 stated {Staff #1} said sorry. Child #1 stated "{Staff #1} hit me too on my head. I cried. It was nap time."
3. Staff #2 stated that she spoke with Staff #1 regarding the incident reported on 11/01/2019 and that Staff #1 told her they were trying to get the Child to stay on the mat.
4. While Child #3 (age 3) was being interviewed about a separate incident, Child #3 stated "{Staff #1} hit me in the bathroom" and stated {Staff #1} hits her with "his tool."

Plan of Correction: The individual was removed from the Columbia Child Development Center and placed on administrative leave. Further disciplinary actions pending receipt of investigation reports of CPS and local police.

Program Directors will maintain increased visibility during program operational hours.
