A monitoring inspection was conducted on 07/18/2023. There were a total of 228 children with 43 staff members present. Children were observed during lunch, on the playground, handwashing, and engaged with staff members. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-70 Description: Based on a review of 10 staff records, the facility did not ensure each staff member had 2 references.
Evidence:
Staff # 1- 6 did not have any documentation of references in their record.
Staff # 7 had 1 of 2 required references in their record.Plan of Correction: The CDC's Licensing Specialist will ensure that all the staff records keep 2 references in each CDC staff file.
The Camp Director will ensure that all the staff records keep 2 references in each Camp Staff File.
Standard #: 8VAC20-780-280-B Description: Based on observation, the facility did not ensure potentially hazardous substances were kept in a locked place.
Evidence:
In room 156 there was an unmarked spray bottle by the back door of the classroom on a shelf.Plan of Correction: Columbia CDC Teachers will continue to be periodically reminded by the Licensing Specialist to label all the substances and keep them in a locked place if this is a hazardous substance.
Each teaching team in each classroom will continue to double check before opening and during the day that all substances are properly labeled and out of reach of children when applicable.
Standard #: 8VAC20-780-550-G Description: Based on a review of documentation and interview, the facility did not maintain documentation of emergency evacuation drills.
Evidence:
The April 2023 fire drill was not documented. The Director confirmed the drill was completed but was not documented.Plan of Correction: Before a drill, the CDC administrative person leading the drill will make sure s/he has the drill log page, and will record the required data on that log immediately. After the drill, the same person will double check that all fields are complete and that the log page is put back in the drills binder.
The Licensing Specialist will routinely review that the logs for every drill are in the drills binder, including shortly after every drill and on a monthly basis.
