Technical Assistance:
Technical assistance given and licensing standards regarding medication authorization and administration were reviewed.

Comments:
An unannounced renewal inspection was conducted on July 25, 2023. The inspector arrived at 12:45pm and departed at approximately 2:45pm. There were 77 children present and 12 staff members. A sample size of 24 records were reviewed which included 8 children?s records, and 16 staff and board member records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during indoor free play, bottle feeding, and nap. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site assistant director at the exit interview.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on record review and interview, it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1. Staff #2 (date of hire May 2021) has an expired tuberculosis test dated May 14, 2021.
2. Staff #1, confirmed that Staff #2?s tuberculosis test is expired.

Plan of Correction: The center responded with the following: Staff #2 has a scheduled appointment to get an updated tuberculosis screening on July 26, 2023. The information of the screening was obtained with negative results and will be placed in the staff member?s record.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that the areas and equipment of the center, inside and outside, shall be maintained in a safe condition.
Evidence:
1. In the 4 year old classroom there is a cracked and broken toy bin, in reach of the children in care, that poses as a pinching hazard.
2. In the 4 year old classroom there is a wooden play closet for the children that has a broken hinge and an exposed screw that poses as an injury hazard to children in care.
3. In the shared 2 year old and 4 year old bathroom, there is a box of disposable gloves on the floor in reach of children in care. The gloves pose as chocking hazards to the children.
4. Staff #1 confirmed that the toy bin and broken play closet, in reach of children in care, are broken. Staff #1 confirmed and acknowledged the box of disposable gloves were on the floor of the bathroom and pose as a choking hazard.

Plan of Correction: The center responded with the following: The toy bin will be replaced and was removed from the classroom during the time of the inspection. A work order will be placed for the wooden play closet and tape was placed on the door to ensure that children would not be injured. The gloves were removed from the bathroom floor during the time of the inspection and placed in a safe area away from children.
