A renewal inspection was conducted on 01/22/2024. There A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed children and staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: (Repeat Violation) Based on a record review the center did not ensure all out-of-state background requirements were met.
Evidence:
Staff #3, hired on 12/05/2023, listed living outside of the state of Virginia within the last 5 years. The center did not have documentation of having sent for the required checks form that state.Plan of Correction: The director will send for the out-of-state background checks this week and document it in the file.
Standard #: 8VAC20-780-70 Description: Based on record review the center did not ensure all required information was kept for each staff person.
Evidence:
1. Staff #1, hired 01/17/2024 and Staff #3, hired 12/05/2023, did not have 1 of the 2 required reference checks
2. Staff #4, hired 12/27/2023 did not have documentation of a job title.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-270-A Description: (Repeat Violation) Based on observation the center did not ensure all areas and equipment of the center, were maintained in a safe condition.
Evidence:
1. In the pre-school classroom the hinge on the door leading to outside, was not secured and dangled over the threshold.
2. In the 2 year-old classroom the glass window of the back door leading outside was cracked in the shape of a square.Plan of Correction: There are work orders already placed for each of the physical plant issue. The director will follow-up with maintenance to determine when it can be repaired.
Standard #: 8VAC20-780-280-B Description: (Repeat Violation) Based on observation the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the school-age room there was a bottle of glass cleaner and 2 sanitizing spray bottles in an unlocked closet.
2. In the infant room there was a bottle sanitizing spray and air freshener in an unlocked cabinet.Plan of Correction: The director is having a staff meeting this week and will address the storage of chemicals with all staff.
Standard #: 8VAC20-780-280-D Description: Based on observation, the center did not ensure all cleaning and sanitizing materials were not be located above food or stored in areas physically separate from food.
Evidence:
In the infant room there was an unlocked cabinet that contained Air freshener, sanitizing spray, infant formula and a bottle hot sauce on the same shelf.Plan of Correction: The director is having a staff meeting this week and will address the storage of chemicals with all staff.
Standard #: 8VAC20-780-290-A-3 Description: Based on observation the center did not ensure all electrical outlets had protective covers.
Evidence:
In the preschool room there were 2 electrical outlets that did not have covers.Plan of Correction: The director will cover the electrical outlets with covers and remind staff to ensure they are covered at all times.
