An unannounced monitoring inspection was conducted in the center on August 16, 2023 from 11:40 am-2:05 pm and concluded on August 22, 2023. At the time of entrance, 62 children were in care with 9 staff members present. The sample size consisted of 5 children?s records and 5 staff records. Children were observed eating lunch, preparing for rest period and during rest period.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-M Description: Based on observation and staff interviews, the licensee did not ensure the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
Evidence: In the Preschool classroom, Child #1 has a food allergy (milk) and was observed in care. Child #1 was not listed on the food allergy/ sensitivity list in the preschool classroom.Plan of Correction: Child #1 has been added to the allergy / sensitivity list in all classrooms and kitchen
Standard #: 8VAC20-780-60-A Description: Repeat
Based on record review and staff interviews, license did not ensure that children?s records contained all required information.
Evidence: The following records were missing information as listed below;
1.Child #1, Child #3 and Child 4 were missing the employment location and phone numbers for both parents listed.
2.Child #1 has a diagnosed food allergy (milk) and there was not a written care plan, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.Plan of Correction: Child #1 notified parent of missing information and information was updated 8-23-23

Standard #: 8VAC20-780-70 Description: Based on a record review and staff interviews, the licensee did not ensure that staff have all required documentation in their record
Evidence:
1.The record for Staff #5 did not include documentation to demonstrate the individual possesses the education and certification required by the job position.
2.The record for Staff #1 did not have documentation of the required 16 hours of annual training as there was only 6 hours of training available for review.Plan of Correction: Staff #5 will provide transcripts that state degree major and minor. by 8-30-23

Standard #: 8VAC20-780-450-A Description: Based on observation and staff interview, the licensee did not ensure cots/mats used by children other than infants during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.
Evidence: In the Preschool Room 3 out of 14 children did not have a bottom cover on their cot.Plan of Correction: Ensure we have linens on all beds. ordered more linens in case of any incidents or accidents that are readily available.
