An unannounced monitoring inspection was conducted from 10:30 am to 11:30 am. At the time of inspector's arrival there was 5 children aged 15 months-3 years, for a total of 12 points, in care with the provider and assistant provider. The records sample size consisted of 5 children's records and 2 provider / household member records. The children were observed engaging in creative play, and interacting with the provider. The provider stated that no medication is administered to children while in her care and there have been no injuries. Areas of noncompliance are detailed in the violation notice and were discussed with the provider during the exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-111-780-B Description: Based on record review and interview it was determined the provider did not review the children's records with the parents annually and obtain the parent's signed acknowledgement of the review.
Evidence:
1. The five children's records reviewed did not contain documentation of annual review and updates if applicable.
2. The provider confirmed that the five records reviewed needed annual updates.Plan of Correction: Provider will have parents review records and document the review and or update at pick up.
Standard #: 22VAC40-111-60-B Description: Based on record review and interview it was determined the provider did not ensure that the record for each enrolled child contained all of the required information.
Evidence:
1. The record for child #1(enrolled 5/10/17) and child #5(enrolled 5/1/17) did not contain the medical insurance policy number for the child.
2. The provider confirmed the record for child #1 and #5 did not contain medical insurance policy number.Plan of Correction: Provider will request medical insurance policy numbers from parents at pick up.
