A Subsidy Health and Safety inspection was conducted on 2/5/20 from 8:00am - 10:15am. During the inspection there were 67 children ages two years old through ten years old in care with seven staff. Children were observed leaving for the local public schools and participating in various activities in the classrooms and eating breakfast. Records were reviewed for seven children and nine staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements, were discussed during the exit interview, and are identified on the violation notice.

Standard #: 22VAC40-665-510-B Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. The attendance record for multiple classrooms did not have the the time of arrival for all children present.
2. Staff #2 confirmed that the arrival of all children was not documented as the attendance in the room just indicates whether the child was present and some parents did not sign their children in at the front desk.Plan of Correction: The facility responded: The time of arrival is usually put on the sign in sheets. we will correct this and make sure parents sign the time in. Vice Principal will monitor and Principal will ensure.
Standard #: 22VAC40-665-780-A-3 Description: Based on a review of documentation and interviews, it was determined that the vendor did not ensure that the center shall practice lockdown procedures at least annually.
Evidence:
1. The emergency drill record for the previous year did not include documentation to demonstrate that a lockdown practice drill was completed.
2. Staff #2 confirmed that a lockdown practice drill was not completed during the previous year.Plan of Correction: The facility responded: Emergency drills/lockdown will be performed once a year as required. Vice Principal will monitor and Principal will records to ensure compliance. A lockdown drill will be held in February 2020..
