An unannounced renewal inspection was initiated and concluded on July 14, 2022. There were 30 children inc are age two and half - ten years old with five staff. Field trip procedures were observed as a group of children left with three staff for an outing. Children, staff and board member records were reviewed. Children were observed in teacher and self-directed activities including but not limited to craft projects, table top toys/games and transitions. A walking tour of the facility inside and outside was completed. Annual inspections, required posting, emergency evacuation drills and emergency equipment were reviewed. Violations regarding recordkeeping, physical plant, transportation and staff qualifications were observed and appear on the violation notice. The violations were reviewed with the owner at the conclusion of the inspection.

Standard #: 8VAC20-780-70 Description: Based on record review the center failed to ensure that each staff record contains all required information.
Evidence:
1 - The record for staff 1 did not contain documentation of emergency contact information.
2 - Staff 3 confirmed that the emergency information was not present in the record of staff 1Plan of Correction: The required information will be added to the staff's record.
Standard #: 8VAC20-780-270-A Description: Based on observation the center failed to ensure that areas are maintained in safe condition.
Evidence:
There is a top loading freezer in a classroom that does not have a child proof latch. This poses an entrapment hazard.Plan of Correction: A childproof latch will be obtained and placed on the top loading freezer.
Standard #: 8VAC20-780-280-B Description: Based on observation the center failed to ensure that hazardous substances are kept in a locked place.
Evidence:
1 - In the 2 - 3 year old room there is hand sanitizer in a cabinet that sits on the floor. The cabinet is unlocked and the sanitizer is accessible.
2 - In large closets/cabinets in the hallways there were bottles of hand and surface sanitizer and bleach.
3 - In the preschool classroom there is small spray bottle of whiteboard cleaning solution.Plan of Correction: All staff will be retrained and reminded that all chemicals must be stored in locked locations at all times when not in use.
Standard #: 8VAC20-780-340-D Description: Based on observation, record review and interview the center failed to ensure that in each grouping of children at least one staff member who meets program leader qualifications is regularly present.
Evidence:
1 - Stall 2 who was alone in the 2 - 3 year old classroom providing direct care to children does not have any documentation that she meets program leader qualifications.
a. The job title of record for staff 2 is teacher assistant.
b. Staff 3 stated that staff 2 was hired as a teacher assistant.
c. Staff 2 has a documented date of hire as July 12, 2021.
2 - Staff 4 who was alone in the school age classroom providing direct care to children does not have any documentation that she meets program leader qualifications.
a. The job title of record for staff 2 is teacher assistant.
b. Staff 3 stated that staff 2 was hired as a teacher assistant.
c. Staff 4 has a documented date of hire September 16, 2021.Plan of Correction: Center management will re-evaluate who goes and stays at the center on field trip days to ensure that a program leader qualified staff is present in each group. Current staff who do not meet program leader qualifications will begin the process to become program leader qualified by completing the required 24 hours of training.
Standard #: 8VAC20-780-580-C Description: Based on observation the center failed to ensure that all children have required safety restraints when being transported.
Evidence:
1 - One seven year old child did not have a booster seat when being transported in the center vehicle.
2 - Staff present stated that the child was eight years old. However review of the child's record determined that he was seven years old and therefore, required a booster seat for transportation.Plan of Correction: Going forward children who require a booster seat will only be transported when a booster seat is available for each child.
