Comments:
A renewal inspection was conducted on 7/7/22 from 10:30 am to 1:20pm. There were 62 children present, in direct care of 10 staff members. The children were observed enjoying outdoor water play, coloring, playing in small centers and eating lunch. The lunch menu include turkey sausage, green beans, roasted potatoes, bread and milk. 3 children' records, 4 staff records, emergency drill records and the center's policies were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-770-60-C-2
Description: Based on records review and staff interview, the center did not obtain results of a central registry search for an employee within 30 days of employment.
Evidence:
1. Staff # 1's first day of employment was 5/23/22.
2. Staff # 3 stated that the form was mailed but the results have not been received yet.
3. There was no written documentation of any follow up related to the delayed search results.

Plan of Correction: If documents are delayed we will be sure to notate communication with agencies for an expected return date. Results that are usually emailed were physically mailed with a post marked date of 7/8/22 and these are in the staff files.

Standard #: 8VAC20-780-130-A
Description: Based on records review, the center did not obtain shall obtain documentation of immunizations the first day of attendance.
Evidence: Child # 2's first day of attendance was 3/7/22. Child # 2's immunization records were not obtained by the center to this date.

Plan of Correction: We will request immunizations from family.

Standard #: 8VAC20-780-130-A
Description: Based on records review, the center did not obtain shall obtain documentation of immunizations the first day of attendance.
Evidence: Child # 2's first day of attendance was 3/7/22. Child # 2's immunization records were not obtained by the center to this date.

Plan of Correction: We will audit files to obtain additional records.

Standard #: 8VAC20-780-130-E
Description: Based on records review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child # 1's record only had initial immunization records from March 2020. Additional immunizations records were not obtained after that date.

Standard #: 8VAC20-780-160-A
Description: Based on records review, the center did not obtain documentation of a negative tuberculosis (TB) screening from each employee by the first date of employment.
Evidence:
1. Staff # 1's first day of employment was 5/23/22. Documentation of a negative TB screening has not been obtained to this date.
2. Staff # 2's first day of employment was 6/2/22. Staff # 2's TB screening was dated 6/19/22.

Plan of Correction: We will ensure TB tests are obtained within the appropriate time frame for all staff and request current results.

Standard #: 8VAC20-780-80-A
Description: Based on record review, observation and staff interview, the center did not maintain written record of daily attendance documenting the arrival of each child as it occurs.
Evidence:
1. At 11:10 am, there were 12 children present in the Discovery Preschool classroom.
2. The Child Supervision Record and the electronic tablet used by the center to record attendance showed that only 11 children were present.
3. Staff # 4 counted the children and confirmed there were 12 children present in the classroom. Staff # 4 stated that Child # 3 arrived at approximately 10:40 am but was not recorded in the attendance.

Plan of Correction: We will retrain staff on accurate attendance tracking and review with staff consistently.

Standard #: 8VAC20-780-570-I
Description: Based on observation and staff interviews, the center did not maintain a one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care.
Evidence: Only 1 of 4 children in the infant classroom using breastmilk and/or formula had a one day emergency supply of commercial formula.

Plan of Correction: We will request that emergency formula be provided for children using breastmilk / formula.
