This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 8/3/2021 and concluded on 8/10/2021. The Director was contacted by telephone at 2:20pm and a virtual inspection was conducted. There were 59 children present, ranging in ages from 3 months to 12 years, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Evidence:
1. Staff #3 (doh: 3/30/2021) had a sworn statement on file dated 4/6/2021.Plan of Correction: Moving forward, all sworn statements will be completed by the first day of employment.
