This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 07/29/2020 and concluded on 07/30/2020. The director person was contacted by telephone to initiate the inspection. There were 42 children present and 6 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-185-130-C Description: Based on record review, the center did not obtain documentation of additional immunizations once between each child's fourth and sixth birthdays.
Evidence:
1) On the date of inspection, the record for Child #2 (dob 10/20/2013), did not contain a current immunization record. The most recent immunization report available in Child #2's record was dated 10/21/2014.Plan of Correction: The center shall obtain a current immunization report for Child #2.
Standard #: 22VAC40-185-160-C Description: Based on record review, not each staff member submitted documentation of a negative tuberculosis screening at least every two years from the date of the first initial screening or testing.
Evidence:
1) The record for Staff #1 did not contain a current Tb screening result. The most recent Tb screening result available was dated 02/21/2018.Plan of Correction: The center shall obtain a current Tb screening report for Staff #1.
