A monitoring inspection was conducted on 03/27/2023 between the hours of approximately 9:00 am and 11:00 am. There were 27 children present, ranging in ages from 7 months to 5 years, with 7 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-4 Description: Based on record review, the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Sex Offender Registry search request and an out-of-state Criminal History Name Check and that they were obtained prior to the first day of employment.
Evidence:
1) The record for Staff #1 (start date 12/01/2022) did not contain documentation of an out-of-state Sex Offender Registry search request and an out-of-state Criminal History Name Check. Staff #1 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment.Plan of Correction: Criminal history check request completed. Will ensure moving forward that I am using correct forms.
Corrected sex offender search completed. Out-of-state checks will be done prior to start date.
Standard #: 8VAC20-780-240-B Description: Based on record review, staff did not complete required orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence:
1) The record for Staff #3 (start date 02/13/2023) did not contain documentation that the employee completed orientation training as required.Plan of Correction: Orientation training form will be signed and completed before working alone with children. Form was found located in another staff's file.
Standard #: 8VAC20-780-240-D Description: Based on record review, staff were not provided in writing with the center's information listed in 8VAC20-780-420 A and other procedures, prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence:
1) The record for Staff #3 (start date 02/13/2023) did not contain documentation that the employee had received this required information.Plan of Correction: Form was found located in another staff's file. Original was completed on 02/13/2023.
