Comments:
The inspection findings have been updated as a result of a supervisory review.

An unannounced inspection was conducted on 06/24/2024 in response to a complaint received by the licensing office on 06/20/2024 relating to allegations of forbidden actions. This inspection was conducted jointly with Loudoun County Child Protective Services.
The inspector reviewed 1 child record and 1 staff record on-site. This inspection included document review, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/31/2024. A POC submitted after this date will not appear on the public website.

Description: Based on observation of video footage, it was determined that one staff person engaged in forbidden actions with a child such as: roughly handling a child, restricting movement, and forcing a child to assume an uncomfortable position.
Evidence:
1) Video footage of an incident on 06/18/2024, showed Staff #1 sitting on the floor, next to Child #1 (age 2) who was laying on their back on their cot. Staff #1 was observed gently tapping Child #1 on their abdomen. On the time stamp 0:21 of the video, Child #1 was observed getting restless, moving their legs and body. At the 1:05 time stamp, Staff #1 was observed using her hands and legs to hold Child #1 down by their shoulders and legs. This action was repeated several times. At the 1:53 time stamp on the recording, Staff #1 was observed grabbing Child #1 and abruptly turning them over from their back onto their stomach on their cot, Staff #1 then placed their legs on Child #1's legs and held them down and used her hands to hold down
Child 1's shoulders. This continued for approximately 24 seconds (until time stamp 2:17).

Plan of Correction: Staff #1 is no longer employed with La Petite Academy. Staff have been retrained on Naptime Procedure and the Proper Handling of a Child. Management will monitor classrooms at high risk time frames.
