A monitoring inspection was initiated at 10:30 and concluded at 11:45 AM. At the time of the inspection there were 58 children (ages infant through preschool) in care with ten staff. Children were observed participating in center based activities and playing outside. Infant care, diapering and medication were also reviewed. The sample size was two staff and two children's records.

Standard #: 8VAC20-780-130-E Description: Based on a review of two children's records, it was determined that the facility did not ensure that updated immunizations are obtained every six months for children under the age of two years.
Evidence:
The record for child #2, who was under the age of two, had not had updated immunizations for over sox months.Plan of Correction: Center management willl obtain updated immunizations for child #2 and will implement a system to ensure that all children under the age of two obtain updated immunizations every six months.
Standard #: 8VAC20-780-60-A Description: Based on a review of two children's records, it was determined that the facility did not ensure that all required information was present in each child's record.
Evidence:
The record for child #1, who was in care at the time of the inspection, had only one emergency contact person.Plan of Correction: Center management will contact parents to have them provide an additional emergency contact.
