A monitoring inspection was conducted on 04/05/2023. There were 31 children present with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview

Standard #: 8VAC20-780-270-A Description: Based upon observation, the facility has not ensured that outside areas of the center are maintained to be safe.
Evidence:
There is poison ivy growing on the fence in the back corner of the toddler playground in an area accessible to the children.Plan of Correction: The facility responded with the following::
The maintenance department will be contacted to remove all poison ivy from the playground fence.
Standard #: 8VAC20-780-320-B Description: Based upon observation and staff interview, the facility has not ensured that the children's bathroom sinks do not have warm water that exceeds 120 degrees F.
Evidence:
1. The water in the boys restroom in the multi-age classroom reached a temperature at which hands could not remain in the water without being burned.
2. Staff 1 acknowledged that the hot water was too hot for the children's use.Plan of Correction: The facility responded with the following:
Staff were instructed not to allow the children to use the handwashing sink in the boys bathroom until maintenance could reduce the hot water temperature below 120 degrees F.
Maintenance was contacted to adjust the water temperature the day of the inspection.
In the future, routine checks of the hot water temperature will be conducted.
Standard #: 8VAC20-780-500-B Description: Based upon observation, the facility has not ensured that disposable diapers are disposed in a leak-proof or plastic lined storage system that is either foot-operated or used in such a way that neither staff member's hands nor the soiled diaper touches the exterior surface of the storage system during disposal.
Evidence:
Staff 6 was observed using her hands to open the lid on the diaper disposal system when disposing of an infant's soiled diaper.Plan of Correction: The facility responded with the following:
All staff will be retrained to properly change and dispose of soiled diapers and action to take if a staff person is not following instructions when changing a diaper. The lid on the dipper disposal system was not properly attached. The lid was attached properly during the inspection.
Standard #: 8VAC20-780-510-I Description: Based upon observation and staff interview, the facility has not ensured that medication is administered only with written parental permission.
Evidence:
1. Child 5 was administered dextroamphetamine and oxcarbazepine for seven days (March 27, 28,29,30,31 and April 3 &4) without written parental permission.
2. Staff 1 acknowledged that the medication was administered to child 5 and that the parent had not signed the authorization form.Plan of Correction: The facility responded with the following:
The parent was immediately contacted and arrived to sign the parent authorization form during the inspection. MAT certified staff will be reminded to ensure that there is written parent authorization before administering any medication.
