An unannounced complaint investigation was initiated on 05/10/2024 and concluded on 05/28/2024 in response to a complaint received by the licensing office on 05/06/2024 regarding behavioral guidance. Interviews were conducted relating to the allegations on 05/10, 05/20, and 05/28/2024. The information gathered during the investigation does support the allegation, so the complaint is determined to be "valid". Standard #: 8VAC20-780-400-A Complaint related: Yes Description: Based on interview and documentation review it was determined the center did not ensure that staff interacted with children in a manner that promotes the child's physical, intellectual, emotional, and social well-being and growth.
Evidence:
1. Eight of the ten staff interviewed reported that staff #10?s voice tone is loud and aggressive.
2. Management reported that voice tone has been an area that they have been working with staff #10 on.
3. In the record for staff #10 there was a staff coaching form dated 8/23/23 completed by management regarding a parent complaint about staff #10?s voice tone.
4. Staff #10 stated that ?the way I sound, tone and how I present myself comes across as being overly aggressive?.Plan of Correction: Terminated teacher after internal investigation findings.

Standard #: 8VAC20-780-410-1 Complaint related: Yes Description: Based on interview and documentation review it was determined the center did not ensure that staff did not strike or roughly handle children.
Evidence:
1. Six of ten staff interviewed described staff #10 as roughly handling children.
2. In the record for staff #10 there was a staff coaching form dated 9/13/23 completed by management regarding a parent complaint about staff #10 rough handling their child.
3. Two staff members interviewed reported observing staff #10 backhand child #1 in the face on May 6th, 2024.
4. Staff #10 reported in a written statement dated May 6th, 2024 that they had their pointer finger by child #1?s face, pointing at something and when they stood up their finger accidentally ended up getting him by his eye.Plan of Correction: Terminated teacher after internal investigation findings.
