An unannounced monitoring inspection was conducted on 8/17/2023 from 9:25am to 11:25pm. At the time of entrance, 82 children were in care with 9 staff members present. Children were observed playing in learning centers, coloring with markers, playing with playdough, dancing and following the song directions, participating in circle time, washing hands, drawing with stencils, preparing for breakfast, playing with legos, blocks and coloring. A selection of staff and children records, medications, the physical space, evacuation drills, allergy action plans and attendance records were reviewed. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Walls, bulletin boards and several other areas of the programming space was decorated with artwork completed by the children. Interactions between the children and staff were positive. Standard #: 8VAC20-780-270-A Description: Repeat Violation
Based on observation, the areas and equipment inside and outside were not maintained in a clean, safe and operable condition.
Evidence:
1. In Preschool A classroom, the paint is chipping in 3 places near the door that connects to the other classroom, by the window and right of the window.
2. In the Pre-Kindergarten A classroom, the paint is chipping on the back wall near home living and near the cots. A black chair cushion has a large tear on the back corner of the seat.
3. In the TK classroom, the rug has multiple stains.Plan of Correction: Management has placed a work order to have the center painted in areas of
need. Equipment that is not in good condition has been removed from the
classrooms and new equipment has be ordered.

Based on observation, hazardous substances were not kept in a unlocked place using a safe
locking method that prevent access by children.
Evidence:
1. In the Preschool 1 classroom, a Febreze spray was on a higher shelf in the children's bathroom.Plan of Correction: All hazardous substances have been placed in cabinets with proper working
locks. Management have met with and completed the Hazard Communication
Program training with all staff members. Classroom cleaning guides have been
reviewed and reposted. Staff have acknowledged that all chemicals
must be locked and inaccessible to children at all times.
