A monitoring inspection was conducted on 4/14/22 from 10:00am until 1:40pm. At the time of the inspection, there were 65 children in care with 12 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed engaging in morning learning activities, playing on the playground, eating lunch and resting quietly during nap time. Infant feeding, handwashing and restroom procedures and diapering procedures were also observed. First aid and emergency supplies, documentation of emergency practice drills, children's injury reports and required center postings were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A Description: Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. One of the climbing structures on the playground has a large plastic piece on the side that is broken and cracked, creating a pinching hazard and a fall hazard.
2. The center director confirmed that the climbing structure on the playground has broken pieces.Plan of Correction: The center responded with the following: The climbing structure has an emergency work order placed and this will be addressed on or before 5/19/22. A new piece for the structure is on order and Maintenance will be issued another work order to replace it once received by the center.
Standard #: 8VAC20-780-550-D Description: Based upon review of emergency drill records and interview, the center did not ensured that a practice evacuation drill is implemented monthly.
Evidence:
1. There was no documentation of a practice drill for the month of March 2022.
2. The center director confirmed that the emergency drill log does not contain a practice drill for March 2022.Plan of Correction: The center responded with the following: Documentation of emergency drills is current and hanging up as of today 4/19/22. A Code Adam drill was conducted on 4/18/22 and a fire drill was conducted on 4/19/22. Moving forward management will calendar all required drills to ensure compliance. DM will review during all school visits.
Standard #: 8VAC20-780-550-P Description: Based on record review and interview, it was determined the center did not ensure that the written record of all children's serious and minor injuries contained all of the required components.
Evidence:
1. Five out of seven children's injury reports did not contain the date and time when parents were notified of the injury.
2. Four out of seven children's injury reports did not contain staff and parent signatures or two staff signatures.

The center director confirmed that the injury reports did not contain all the required information.Plan of Correction: The center responded with the following: Incident reports will be turned in to the front office each day and a drawer system was added to ensure proper filing. Each teacher was shown how to properly fill out the incident report including the section that notifies parents of the injury including the date and time, on (date). Management will review all incident reports prior to their being presented to parents, to ensure compliance.
