Discussion on standard requirements for staff records and to make a copy of form when send request for background checks.

An unannounced monitoring inspection was conducted September 11, 2023 approximately 9:30 am to 10L43 am. There were 52 children directly supervised by 10 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. No new-medications on-site since the last visit. Children were observed participating in group play, reading books indoor outside, and eating snack. Observation of handwashing by children and staff. There was an assortment of age appropriate materials for the children in care. The center was clean and organized.

Standard #: 22.1-289.035-B-4 Description: Based on staff records review, 3 out 5 records, the authorize the child day center did not obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #1 (date of employment 8/21/2023) did not have documentation a request was sent for a search of the child abuse and neglect registry for the state of Virginia.
2. Staff #2 (date of employment 8/14/2023) did not have documentation a request was sent for a search of the child abuse and neglect registry for the state of Virginia.
3. Staff #4 (date of employment 8/22/023) did not have documentation a request was sent for a search of the child abuse and neglect registry for the state of Virginia.Plan of Correction: Will keep proof of sent background checks in staff files. Will send completed documentation on first day of employment.
