Comments:

A monitoring inspection was initiated on 09/09/2021 and concluded on 09/15/2021. The Program Director was contacted by telephone to initiate the inspection. The Program Director reported that the current census was (28) twenty-eight active foster child placements, (33) thirty-three active foster home providers and (4) four newly hired staff since the last inspection. The Inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The Inspector reviewed (3) three complete foster child records, (5) five foster home provider records and (5) five employee records. An exit interview was conducted with agency representatives on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Based on the review of the file for Foster Child (FC) 2, the licensee failed to document a medical examination report that included a signature and title of the examiner. Findings: The report of physical examination form in the record for FC2 did not contain a signature and title of the examiner.

Description:	Based on the review of the file for Foster Child (FC) 2 and based on an agency representative interview, the licensee failed to maintain a copy of a dental examination report in the record for FC-2. Findings: 1.At the time of the inspection, there was no dental examination report in the record for FC-2. 2.During the exit interview, AR-8 reported transporting this child to a dental evaluation however, this staff reported that the dental examination report was not obtained.
