Type of inspection: Renewal This inspection was initiated at license location. Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 2/29/2024 Onsite licensed location from 10:40 a.m. to 3:33 p.m. 3/1/2024 EHR remote access from 10:30 a.m. to 5p.m. 3/4/2024 EHR remote access from 9:30 a.m. to 5 p.m. The Acknowledgement of Inspection form was reviewed, signed and left with Director of Youth Services/Executive Director on 2/29/24. Number of children in care: 27 Number of approved provider homes: 28 Number of provider records reviewed: 3 Number of staff records reviewed: 3 Number of background checks only: 5 Number of children records reviewed: 3 Number of discharge records reviewed: 2 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Observations by the licensing inspector: Posting of the license and inspection documents was observed. An exit meeting was conducted on 3/6/2024 at 1:30 p.m. at licensed office location.

Description:	Violation: Based on review of record for staff (2), agency failed to complete criminal history record check within 30 days of employment or volunteer services. Findings: 1. S (2) was hired by child placing agency 8/16/2023. 2. Sworn statement in record for Staff S (2) was dated 8/16/2023. 3. Department of Social Services Central Registry results in employee record were dated 9/4/23. 4. Record for Staff S (2) did not have evidence of a criminal history report in record within 30 days of employment or volunteer service. VA State Police criminal history document in record for S (2) was dated 2/29/24. 5. The findings was reviewed during the exit meeting that occurred on 3/6/2024. Staff S (7) provided an explanation for the findings identified by the licensing inspector for Staff S (2) and acknowledged findings.
