Type of Inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 12/12/2024 from 10:00 a.m. to 2:41 p.m. The licensing Inspector met the Executive Director at the license location to initiate the inspection. . The Acknowledgement of Inspection form was signed and left at the facility for each day of the inspection. Number of children in care: 22 Number of approved provider homes: 24 Number of children?s records reviewed: 2 Number of provider records reviewed: 2 Number of staff records reviewed: 2 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Posting of the license was observed and an exit meeting was conducted on 12/12/2024 with Executive Director, Human Resource supervisor, Placement supervisor and Resource Home supervisor. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
