Comments:
A renewal inspection was initiated on September 15, 2021 and concluded on September 20, 2021. The Chief Compliance Director was contacted by telephone to initiate the inspection. The Compliance Director reported that the current census was twenty-three (23) foster children and twenty-four (24) foster home providers. There were two (2) newly hired staff since the last inspection.

The inspector emailed the Compliance Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed five (5) board officer records, three (3) foster child records, two (2) complete foster home provider records and two (2) staff records submitted by the facility to ensure documentation was complete. An exit interview was conducted with agency representatives on September 20, 2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or laws, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-131-250-N
Description: Based on the review of the records for Foster Child (FC)-1 and FC-2 and based on an interview with agency representatives (AR), the licensee failed to document a summary of the pre-placement interview and results of the pre-placement visit in the child's file.
Findings:
1. The records presented did not document a summary of the pre-placement interview and results of the pre-placement visit in the file for FC-1 & FC-2.
2. AR-1 and AR-2 referenced a date of the pre-placement interview and pre-placement visit which was documented in the social history for FC-1 and FC-2. However, both AR-1 and AR-2 acknowledged that while the date was referenced there was no documentation of a summary of the pre-placement interview and results of the pre-placement visit in the child's file.

Standard #: 22VAC40-131-340-F-1
Description: Based on the review of the records for Foster Child (FC)-1 and FC-2 as well as based on an interview with agency representatives (AR), the licensee failed to document the involvement of the birth parents in the development of the individualized service plans.
Findings:
1. Based on the records for FC-1 and FC-2, the birth parent's rights were not terminated, the birth parents were not deemed inappropriate and were available to participate in the development of treatment planning.
2. Based on the records, the Individualized Service Plans documents that the birth parents were not involved in the development of the treatment plan.
3. During the exit interview, AR-1 and AR-2 acknowledged that they did not actively involve the birth parents in the development of the treatment plans.
