Type of Inspection: Monitoring
On 6/11/2024 Licensing inspector initiated monitoring inspection. The Program Director was contacted by telephone to initiate the monitoring inspection. The Program Director reported that the current census was (25). The inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed (#2) resident. records, (2) staff records, and (2) provider home records submitted by the facility/ to ensure documentation was complete. Provider has not had any updates to policies and procedures since last inspection and hired two new staff since last inspection.
An exit interview was conducted with Executive Director, Program Director, Compliance supervisor, case worker supervisor and compliance manager on 6/24/2024 via TEAMS, where findings were reviewed, technical assistance was provided and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations documented on the violation notice issued to the facility.
Licensing specialist discussed process for initiating and submitting facility's background request. for employees with agency representatives during the exit meeting on 6/24/24. There were no changes to facility's policies or procedures since the last inspection.

2. Sworn statement in the record for employee S#2 was completed and dated on 4/29/2024.
3. The findings were reviewed during the exit meeting and acknowledged by agency representative.Plan of Correction: Human Resources will provide our offered candidates the sworn statement at the time of initial application.
