Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on April 20, 2021 and concluded on April 23, 2021. The Program Manager was contacted by telephone to initiate the inspection. The Program Manager reported there were five (5) children in care and five (5) approved foster homes. No new board officers were reported. The inspector emailed the Program Manager a list of items required to complete the inspection. The inspector reviewed one child?s record, one foster home record, and a portion of one personnel record submitted by the agency to ensure documentation was complete. Exit meeting was held with the Program Manager on April 23, 2021. The agency reports they are following their COVID-19 policies and procedures. Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the agency. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return to the Licensing Office within five (5) business days from receipt of the inspection documentation. You will need to specify how the deficient practice will be or has been corrected (merely writing the word corrected is not acceptable). Your plan of correction must contain the steps to correct the noncompliance with the standard(s), the measures to prevent the noncompliance from occurring again; and the position responsible for implementing each step and/or monitoring any preventive measure(s). The licensee should retain a copy to be posted at the agency. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Description:	Violation: Based on review of the record for foster home FH1 and interview with Staff S1, the date received was not documented on a sworn statement in the record. Findings: 1. The sworn statement for household member, HH1, was reviewed. The sworn statement did not document the date received by the licensee. 2. Staff S1 confirmed that the sworn statement did not include the date received

Description:	Violation: Based on review of the record for foster home, FH1, and interview with Staff S1, the licensee failed to ensure the home study, dated 10/10/2020, documented required information. Findings: 1. The home study for FH1 failed to document assessments of the following required components: - 180-J-1.c. S1 reported that an interview was conducted with Foster Parent A on 10/09/2020 regarding the driving record convictions. However, this assessment was not included in the home study. . - 180-J.2.e.4. the home study did not include documentation that the foster parents understand they are prohibited from using any of corporal punishment on the child and also prohibited from giving permission to others to use any form of corporal punishment on the child - 180-J.2.e.10. the home study did not document that the foster parents have in-force vehicle liability insurance 2. S1 acknowledged that the home study did not address the above required components.

The sworn statement was signed but not dated. 2. S1 acknowledged the date was missing from the sworn statement.
