Comments:
An unannounced renewal inspection was conducted on 1/14/2020 from approximately 9:40 am to 4:15 pm and 1/15/2020 from 9:15 am to 4:35 pm at the Richmond location of United Methodist Family Services (UMFS). Personnel records were reviewed on 12/10/19 from 10:20 am to 2:15 pm.

The agency reports 63 approved provider homes and 53 children in care. During this inspection, seven provider home records and six foster child records reviewed. Five additional provider home records were reviewed for compliance with background checks. Nine personnel records were reviewed. Background checks for the board officers were reviewed on 12/10/2019 and are in compliance.

The human resources recruitment and talent manager and human resources coordinator were present for the exit interview to discuss the findings for the personnel records which took place on 12/10/19 from 2:00 pm to 2:15 pm.

The regional director and assistant regional director were available for the inspection and participated in the exit interview to discuss preliminary findings for the provider homes and children records which commenced at 4:00 pm on 1/15/2020. The assistant director of quality improvement and risk management participated in the exit interview on 1/15/2020 and the human resources recruitment and talent manager participated in further discussion of inspection findings for one personnel record during the 1/15/2020 exit meeting.

There were three violations of the Standards for Licensed Child Placing Agencies. See the violation notice on the Department?s public web site for violation of the Standards. An acknowlegment of inspection form was left with the regional director on 1/15/2020.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:
Based on a review of the record for Personnel (P) P8 and interview with the HR recruitment and talent manager, the licensee failed to ensure a copy of the educational degree was documented in the record.

Findings:
P8 was hired on 9/23/19 as a clinical support specialist. The job description for a clinical specialist requires a Master's or Bachelor's degree in social work or a related human service discipline and six months of experience or combination of education and experience. Based on an interview with the HR recruitment and talent manager P8 was hired based on his experience and a copy of the educational degree was not obtained.

Plan of Correction: HR Manager has requested a copy of the degree from the employee to be produced by 1/31/20.

Description: Violation:
Based on a review of the record for Foster Home (FH) FH1 and interview with the assistant director, the licensee failed to obtain a signed authorization form allowing the previous child placing agency to release information to the licensee.

Findings:
The home study in the record for FH1 documents prior approval with two licensed child placing agencies. The record failed to document a signed release form for one of the previous agencies.During an interview with the assistant director, it was acknowledged that this was an oversight.

Plan of Correction: Assistant Director will retrain social workers on how to properly complete releases with other TFC agencies by 1/31/20.

Standard #: 22VAC40-131-340-E-1
Description: Violation:
Based on a review of the record for Foster Child (FC) FC3 and interview with the regional director, the licensee failed to include an assessment of FC3's dental needs in the individualized service plan.

Findings:
FC3 was placed on 6/4/19. The individualized treatment plan completed on 7/4/19 does not include an assessment of FC3's dental needs. During an interview with the regional director, it was acknowledged that this requirement was overlooked.

Plan of Correction: Social worker will add an addendum to the treatment plan regarding dental assessment by 1/31/20. Social workers and supervisors will review licensure standards regarding individualized services plans, to include assessments of all needs categories, by 1/31/20.
