This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An announced monitoring inspection was initiated on 3/10/2021 and concluded on 3/15/2021. The regional director was contacted by telephone to initiate the inspection. The agency reported 40 children in care and 50 approved provider homes.
The inspector emailed the regional director a list of items required to complete the inspection. The regional director was available for questions throughout the inspection.
The inspector reviewed 4 provider home records, 4 children records and 4 personnel records. Background checks for 3 additional provider homes were reviewed. An exit interview was conducted by telephone on 3/15/2021 from approximately 10:00 am -10:35 am with the regional director and assistant director of quality improvement and risk management to discuss preliminary inspection findings. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency.
There was 2 citations for violation of the Standards for Child-Placing Agencies.
Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #: 22VAC40-131-290-K-2 Description: Violation:
Based on review of the record for Provider Home (PH) PH5 and interview with Agency Representative (AR) AR1, the licensee failed to ensure the results of the tuberculosis (TB) screening or test date was no older than 13 months at the time of the home approval.
Findings:
Provider Home (PH) PH5 was approved by the licensee on 12/7/2020. The face sheet for PH5 documents two biological children reside in the home.
The TB screening or test for Birth Child (BC)BC1 is dated 5/17/19, which is beyond 13 months of PH5's approval. The TB screening or test for Birth Child (BC)BC2 is dated 1/21/19, which is beyond 13 months of PH5's approval.
During the exit interview, AR1 acknowledged the TB test or results were out of compliance.Plan of Correction: UMFS will get updated TB tests for both children in the Resource Home by 4/15/21.
Standard #: 22VAC40-131-340-F-1 Description: Violation:
Based on review of the record for Foster Child (FC) FC2 and interview with Agency Representative (AR) AR1, the licensee failed to document the involvement of FC2's birth parents in the development of FC2's service plan and quarterly progress report.
Findings:
FC2 was placed with the licensee on 10/26/20. The individualized service plan completed by the licensee documents "No" biological parent involvement. The quarterly progress reports completed by the licensee fails to document the involvement of FC2's birth parents.
During the exit interview, AR1 acknowledged the birth parent involvement was not documented.Plan of Correction: For previous reports on this case, UMFS social worker will write an addendum to the report to clarify birth family participation in treatment planning by 4/1/21. For future reports, birth family participation in treatment planning will be clearly stated
