22VAC40-191 Background Checks for Child Welfare Agencies

Comments:
An unannounced renewal inspection took place on January 24, 2022 from 10:00 am to 3:30 pm and on January 25, 2022 from 10:10 am to 5:15 pm, at the office of United Methodist Family Services (UMFS) in Richmond Virginia. The agency reports 44 approved provider homes and 35 children placed. During this inspection, 5 provider home records, 4 children records and 12 personnel records were reviewed.

The Regional Director was available throughout the inspection. The Regional Director, Community Based Services Supervisor, Associate Director, VP of Strategy and Innovations and HR Manager participated in the exit interview to discuss preliminary findings, which commenced at 4:10 pm on 1/25/2022. An acknowledgement form was left with the Regional Director on 1/25/2022. An additional interview took placed on 2/17/2022 at approximately 4:35 pm with the Regional Director to discuss the final findings for this inspection.

There were two citations for violations of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.
Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:
Based on review of background checks for Staff (S) S1 and interview with S1, the licensee failed to ensure the required criminal history check was completed within 30 days.

Findings:
S1 became a contractor with the licensee on 11/9/2020. The criminal record check for S1 is dated 4/9/2021-beyond 30 days of S1 becoming a contractor with the licensee.
