Comments:
A monitoring inspection was initiated on 10/29/2024 and concluded on 10/292024. The director was available to initiate the inspection. The director reported that the current census was 24 youth and 30 provider homes. Three staff records and three youth records were reviewed. An exit meeting was conducted with the Director, Administrative Services Manager, Site Supervisor, Community Based Supervisor, and Family Systems Coordinator on 10/29/2024, where findings were reviewed and an opportunity was given for questions as well as for providing any information or documentation that was not available during the inspection.

Observations by licensing inspector: The license was posted along with inspection documents.

The evidence gathered during the inspection determined non-compliances with an applicable standard or law, and
a violation was documented on the violation notice issued to the facility. The licensee has the opportunity to submit a
plan of correction to indicate how the cited violation will be addressed in order to return the facility to compliance and
maintain future compliance with the applicable standard or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the
standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any
names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit
interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental
information), you may request a review and discussion of these findings with the inspector?s immediate supervisor. To
make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that
serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS
public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
The department?s inspection findings are subject to public disclosure.

Violations:
Standard #: 22VAC40-131-290-G
Description: Based on a review of a child?s record (FC-1) and interviews with agency representatives, the agency failed to maintain documentation regarding the arrangement of medical care and receipt of care as required. Evidence:

1) A physical contained in the record of FC- 1, dated 11/28/23, indicated that FC-1 suffered a severe injury prior to placement with the agency that required hospitalization from 10/30/23 to 12/6/23.

2) The physical recommended outpatient care to include OT, ST, PT, surgery, GI follow up and rehab.

3) The record did not contain documentation of arranged medical care or receipt of care from 12/6/23 to 8/21/24.

4) The agency was afforded the opportunity to provide evidence to support that medical care for FC- 1 was arranged and received during that timeframe.

5) The findings were discussed during the exit meeting and acknowledged by agency representatives.

6) In a follow- up meeting with an agency representative on 12/2/24, the representative provided medical information dated for April 2024 and November 2024; however, it did not address the specific care regarding FC-1?s injuries.
