An unannounced mandated renewal inspection was completed on 12/23/2019 and 12/30/2019. The on site inspection on 12/23/19 took place from from 10:00 am until 2:00 pm. The on site inspection on 12/31/19 took place from 10:00 am until 1:30 pm. The Datz Foundation provides home study and post-placement supervision services for domestic and international adoptions and parental placement services. Datz also has a domestic agency adoption program. The agency is currently working with four infants in their custody. The Executive Director was present for the inspection and the Contract Social Worker was available for the inspection and the exit interview at the end of the second day. Three adoptive home records reviewed and one birth parent/child record was reviewed. Seven personnel records were reviewed. Background checks for the corporate officer were also reviewed. There were two citations for violations of the Standards for Child Placing Agencies. Both were key health and safety standards.
Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five days, regardless of whether or not the Plan of Correction has been completed.

Standard #: 22VAC40-131-150-D Description: Violation:
Based on a review of the personnel record for P-7, and an interview with an agency representative, the agency failed to ensure that the employee received annual training.
Evidence:
1) The most recent training for P-7 documented in the personnel file is dated 11/09/17.
2) The agency representative acknowledged that training documentation was not contained in the record.Plan of Correction: The Executive director will ensure that annual training is completed and maintained in each personnel file.
Standard #: 22VAC40-131-260-B Description: Violation: Based on a review of the Birth Parent/Child file (BP/Child-1) and an interview with an agency representative, the agency failed include required elements in the child's social history.
Evidence:
1) The social history for BP/Child-1 did not document information on the child's family's structure and relationships.
2) The social history did not document the medical and psychiatric history of the child's parents, siblings, aunts, uncles, and grandparents as it relates to the suitability of the child for placement.
3) The agency representative acknowledged that the social history did not contain the required information.Plan of Correction: The Executive Director will ensure that required information is documented in each child's social history.
