Comments:

Type of Inspection: Monitoring On 6/24/2024 Licensing inspector initiated a monitoring inspection. The District Supervisor was contacted by telephone to initiate the monitoring inspection. The District Supervisor reported that the current census was (4). The Licensing Inspector emailed the District supervisor a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed (1) child record, (2) staff records, and (2) adoptive home records submitted by the agency to ensure documentation was complete. Agency has not had any updates to policies and procedures since last inspection. An exit interview was conducted with the District Supervisor on 6/27/2024 via TEAMS, where findings were reviewed, technical assistance was provided and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations documented on the violation notice issued to the facility. Number of children in care: 4 Number of approved adoptive homes: 17 . Number of children?s records reviewed: 1 Number of adoptive home records reviewed: 2 Number of staff records reviewed: 2 Number of interviews conducted with staff: 1 entrance and exit interview with District Supervisor. The evidence gathered during the inspection determined violations with applicable standard(s) or law. Description:	Based on review of Staff record (SR#1), sworn statement submitted for review to the licensing inspector did not provide evidence that the sworn statement was completed prior to Staff #1 first day of employment at the facility/agency. 1. Staff #1, date of employment with the facility/agency was 4/29/2019. 2. Sworn statements in the record for employee S#1 was completed and dated on 4/29/2019 and 3/9/2022. 3. Staff #2, date of employment with the facility/agency was 2/20/2024. 4. Sworn statements in the record for employee S#2 was completed and dated on 2/20/2024. 5. The findings were reviewed during the exit meeting and acknowledged by agency representative.
