Provider Homes Children?s Services Background Checks Treatment Foster Care

Comments:

An unannounced monitoring inspection was conducted on 5/4/2022 from 2:45 p.m. to 5:30 p.m. An entrance conference and tour was conducted with the Program Director. The agency reported nine (9) foster children in placement and six (6) approved foster homes. No new staff were hired since the last inspection. The inspector reviewed one (1) foster child and one (1) provider home records. An exit interview was conducted with the Program Director on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation Based on record review, the licensee failed to address required elements in the home study for Foster Homes 1 (FH1). Findings: 1) The home study for FH1 did not include a date with the signature of the Executive Director?s designee. 2) The findings were discussed during the exit interview. Staff 1(S1) reviewed the home study. 3) Staff 1 (S1) acknowledged the findings.

Description:	Violation Based on record review, the agency failed to address required elements in the written assessment, as required by 22VAC40-131-250.O., for Foster Child 1 (FC1). Findings: 1. The written assessment, titled Intake Assessment, for FC1 did not include the date of planned achievement for the child?s permanency plan goal as required by 22VAC40-131-250.G.3.i 2. The findings were discussed during the exit interview. Staff 1 (S1) reviewed the written assessment. 3. S1 acknowledged the findings.

Description:	Violation Based on record review and interview, the licensee failed to address required elements in the Social History for Foster Child 1 (FC1). Findings: 1) The Social History for FC1 did not include documentation of parents, grandparents, aunts, and uncles? occupational history. 2) The Social History for FC1 did not include documentation of aunts or uncles? educational history. 3) The Social History for FC1 did not include documentation of aunts, uncles, or grandparents medical or psychiatric history as it relates to the suitability for the child?s placement. The case record documented the paternal grandmother had petitioned for custody of FC1 and the maternal grandmother was a prior caretaker. 4) The findings were discussed during the exit interview. Staff 1 (S1) reviewed the Social History. 5) S1 acknowledged the findings.
