Type of inspection: Monitoring
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection:
4/12/2023 from 1:45 p.m.to 6:00 p.m. A preliminary findings review was conducted on the date of inspection.
The Acknowledgement of Inspection form was completed on the date of inspection.

Number of children in care: 10
Number of approved provider homes: 9
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

An exit meeting was conducted on 4/20/2023 by Teams Meet.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Description: Violation:
Based on record review, the agency failed to obtain annual medical examination reports for Foster Child 1 (FC1).

Findings:
1) The record for FC1 contained a medical examination reporting documenting an exam on 2/22/2022.
2) The record for FC1 did not contain another medical examination report addressing required elements.
3) The findings were discussed during the preliminary findings review.
4) Staff reviewed the record and did not locate an annual medical examination report containing required elements. Staff reported that the physician's office indicated another annual physical could not be scheduled until June 2023 due to Medicaid billing.
5) Staff acknowledged the findings during the preliminary findings review.
6) The findings were reviewed during the exit interview and Staff 1 (S1) acknowledged the findings.

Description: Violation:
Based on record review, the agency failed to obtain a written medical examination report for Foster Child 3 (FC3) that addressed all required elements.

Findings:
1) A Physical Examination Form dated 11/1/2022 for FC3 did not address auditory acuity, nutritional status, or chronic conditions and disabilities.
2) The findings were reviewed during the preliminary findings review.
3) Staff reviewed the Physical Examination Form dated 11/1/2022 and acknowledged the findings.
4) The findings were reviewed during the exit interview and Staff 1 (S1) acknowledged the findings.

Description: Violation:
Based on record review, the agency failed to obtain Sworn Disclosure statement for Applicant 1 (A1) in Foster Home 1 (FH1) that documented it was obtained prior to approval.

Findings:
1) The record for FH1 contained Sworn Disclosure Statements for both Applicants.
2) The Sworn Disclosure Statement for Applicant 1 did not include a date that Applicant 1 signed the form.
3) The Sworn Disclosure Statement for Applicant 1 did not contain a date received.
4) The findings were discussed during the preliminary findings review.
5) Staff reviewed the Sworn Disclosure Statement for both Applicants and acknowledged the findings.
6) The findings were reviewed during the exit interview and Staff 1 (S1) acknowledged the findings.
