Type of inspection: Renewal A monitoring inspection was conducted on 10-23-24 from 11:00 am to 2:00 pm. The Executive Director was available throughout the inspection. The exit meeting was conducted on the same day of the inspection. An Acknowledgment of the Inspection form was left at the location. Number of youth: 1 (recently adopted) Number of approved provider homes: 2 (Note: the families are working with Adoption Services for placement services) Number of youth records reviewed: 1 Number of staff records reviewed: 2 Number of provider homes reviewed: 2 Number of interviews conducted with staff: 1 The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
