This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on August 31, 2020 and concluded on September 8, 2020. The Therapeutic Foster Care Program Director was contacted by telephone to initiate the inspection. The agency reported 25 children in care, and 33 approved provider homes. The agency reports no new staff since the last inspection. Board Officer Records were reviewed and determined to be in compliance at this time. The inspector emailed the Program Director a list of items required to complete the inspection. The inspector reviewed 3 complete children records, 4 complete provider home records, 10 additional provider home records (background and medical records only), and 2 staff records submitted by the agency to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #: 22VAC40-131-180-J-2 Description: Based on the review of the record for foster home (FH)-2 and based on an interview with agency representatives (AR), the licensee failed to ensure that a weapon was maintained in a locked closet or cabinet.
Findings:
1.The ?facilities checklist? for FH-2 dated 12/16/2020 documented a sword that is kept ?in a backroom in an unrecognizable area.? There was no documentation that the sword was in a locked closet or cabinet. The home study dated 01/23/2020 does not mention that there is a sword in the home.
2.During the exit interview, AR-2 reported that there is was an addendum that was not presented to the licensing representative for review in the original documentation. AR-2 presented the addendum during the exit interview. This addendum is dated 01/20/2020, prior to the home-study.
3.The addendum dated 01/20/2020 states that there is a ?knife? kept in a locked case in the back of the closet. However, the home study dated 01/23/2020 does not mention a sword or a knife.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-131-290-K-1 Description: Based on the review of the file for Foster Home (FH-1) and based on an interview with agency representatives (AR), the licensee failed to obtain from the provider applicant documentation on the appropriate report of tuberculosis form a statement that the individual is free from communicable tuberculosis.
Findings:
1.The documentation presented for FH-1 contained a ?physical examination? form dated 11/06/2019 which contained a recommendation from the examining physician that recommended that the foster parent obtain clearance from the health department due to a positive tuberculosis test result.
2.As of the date of the inspection, there was no evidence located in the documentation presented by the licensee or presented during the interview that documents a clearance from the health department.
3.During the exit interview on 09/08/2020, the licensee presented an email from FH-1, dated 09/08/2020 stating that the foster home provider contacted the health department however, there was no documentation of the clearance from the health department.
4.During the exit interview, AR-1 and AR-2 confirmed that there was no documentation from the health department clearing this foster home provider due to the positive tuberculosis testing.Plan of Correction: Not available online. Contact Inspector for more information.
