22VAC40-131-140-F & 22VAC40-131-160-B.2: The licensee is encouraged to examine current practices and ensure documentation of details substantiating qualifications required for identified staff positions in personnel files that are onsite.

Comments:

A renewal inspection was initiated on November 05, 2021 and concluded on November 10, 2021. The Clinical Director was contacted by telephone and email to initiate the inspection. The Clinical Director reported the current census was twenty (20) active child/youth placements, thirty-two (32) active provider homes and two (2) newly hired staff since the last inspection. The Inspector emailed the Clinical Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two (2) active child/youth placement records, four (4) complete provider home records (including one re-evaluated home record), three (3) personnel records. The Inspector reviewed applicable updated policies and procedures as well as staffing information. An exit interview was conducted with agency representatives on the date of the inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
