Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 11/21/2022 from 9:45a.m. to 4:00 p.m. and 11/22/2022 from 11:30 to 2:00p.m. The Licensing Inspector met with the Program Director and the Clinical Director to initiate the inspection and concluded an exit with the Program Director and the Program Assistant. There was a change in staff during this period as the State Director position is vacant. Number of approved provider homes: (16) Number of children in care: (17) Number of children?s records reviewed: (3). Number of provider home records reviewed: (2). Number of staff records reviewed: (2). Including the Board of Directors. Observations by licensing inspector: posting of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection with the Program Director and the Program Assistant. There were no major changes made to the policies or procedures since the last inspection and changes made were included in the renewal packet. The location of the agency plans to modify the license to move to a new office location. The documents have been submitted with supplemental attachments for review. The evidence gathered during the inspection determined (0) violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
