Type of inspection: Monitoring
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 8/7/2023 from 10:30a.m. to 4:00 p.m. and 8/8/2022 from 10:00 to 12:00p.m. The Licensing Inspector met with the Program Director at the site to initiate the inspection.

Number of children?s records reviewed: (2). Number of provider home records reviewed: (2).
Interviewed Social Service Worker regarding caseload duties, supervision, and training. (AR-3)
Number of staff records reviewed: (2) new staff. Observations by licensing inspector: posting of the license and inspection documents was observed. An exit meeting was conducted on 8/8/2023 the date of inspection with the Clinical Director and the Quality Management Specialist. There were no major changes made to the policies or procedures since the last inspection.
The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. Based on the record review, the agency failed
to document in the home study that the home complies with standards for the possession of weapons (e.g. knives) as identified in the home environment checklist.
1) The physical facility checklist completed on 2/21/2023 revealed that knives were locked in a safe and in the foster parent?s closet.
2) The home study narrative dated 3/31/2023 did not document the information obtained from the physical facility checklist.
3) The findings were discussed with the Program Director and during the exit
interview and the staff acknowledged the
