Type of inspection: Renewal
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 2/27/2023 from 1:00a.m. to 5:30 p.m. The Licensing Inspector met with the Director to discuss the entrance to the inspection. The exit interview was held on 3/2/2023 with AR-1.
Number of approved provider homes: (13)
Number of children in care: (9)
The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: (2). Number of provider home records reviewed: (1)
Number of staff records reviewed: (2) during this inspection. Observations by licensing inspector: posting of the license and inspection documents was observed. An acknowledgement of inspection was left with the licensee on the date of inspection. There were no changes made to the policies or procedures since the last inspection.
The Licensing Specialist interviewed the Director (AR-1) regarding program evaluation, services, census and staffing. There were changes to the staffing and capacity and the licensee was asked to make changes to the renewal application submitted.
The evidence gathered during the inspection determined (3) violations with applicable standard(s) or law.

Description: Violation:
Based on the record review, the agency failed to document emotional and psychological needs in the written assessment for (FC-1) and (FC-2).
1) (FC-1) and (FC-2) were admitted on 1/20/2023
2) The referral/written assessment did not contain emotional and psychological needs.
3) The Licensing Specialist asked (AR-1) if the information could be located
4) The findings were discussed during the preliminary findings review and the staff acknowledged the findings.
5) The findings were reviewed during the exit interview.

1) (FC-1) was admitted on 1/20/2023
2) The record did not contain a follow-up eye exam recommendation per physician report dated 6/28/2022 for the child.
3) The Licensing Inspector requested the eye exam follow-up record for (FC-1)
4) The Licensing Specialist asked (AR-1) if the information could be located
5) The findings were discussed during the preliminary findings review and the staff acknowledged the findings.
6) The findings were reviewed during the exit interview.

3) The Licensing Inspector requested the clothing inventory and/or receipts documentation for (FC-1) and (FC-2).
