Type of Inspection: Monitoring On 6/26/2024 Licensing inspector initiated a monitoring inspection. The Executive Director was contacted by telephone to initiate the monitoring inspection. Current census is (0) as this agency does not provide placement services. The Licensing Inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed (1) staff record for two positions, and (2) provider home records submitted by the agency to ensure documentation was complete. Provider has not had any updates to policies and procedures since last inspection. An exit interview was conducted with the Executive Director on 6/27/2024 via TEAMS, where findings were reviewed, technical assistance was provided and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined compliance with applicable standards or law, and violations documented on the violation notice issued to the facility. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
