Comments:

Type of inspection: Renewal An onsite inspection was conducted on 10/17/24 and Executive Director was accessible during the inspection. The Licensing Specialist initiated a request for an inspection with the Executive Director at 11:30 a.m. The Licensing Inspector discussed the census request and the files to be reviewed. An exit interview was conducted with the Executive Director on 10/17/24 and opportunity was given for questions. The Acknowledgement of Inspection form was completed and left at the office location. Number of children in care: N/A Number of approved provider homes: 19 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: N/A Number of staff records reviewed: 1 Number of interviews conducted with staff: 1 entrance interview and 1 exit interview with the Executive Director. Observations by licensing inspector: The license was posted along with required inspection documents. Additional Comments/Discussion: 2 adoptive home records were reviewed. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
