An on site, unannounced, monitoring inspection was conducted on January 26, 2022 from 10:00 am to 2:30 pm. The remainder of the inspection was completed virtually on January 28, 2022 from 10:00 am to 2:00 pm. A virtual exit interview was conducted on 2/01/22 with the Vice President of Community Based Services, the Director of Quality Assurance, the Director of Foster Care Services, and the Assistant Treatment Foster Care Supervisor. The agency reports 17 children currently placed and 24 approved providers.
During this inspection, the new office space was observed for compliance with regulations regarding office space and conditions and two foster child records were reviewed. Three foster home records were reviewed. Background checks for four additional foster home approved since the last inspection were also reviewed and six personnel records were reviewed. There were two citations for violations of the Standards for Child-Placing Agencies. They were key, health and safety standards. The Vice President of Community Based Services, the Director of Quality Assurance, the Director of Foster Care Services, and the Assistant Treatment Foster Care Supervisor were available for the inspection and present for the exit interview on 2/01/22.
Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Standard #: 22VAC40-131-250-G Description: Violation: Based on reviews of the Biopsychosocial and Intake Assessments for two foster children (FC-1 and FC-2) and interviews with agency representatives, the agency failed to include elements in the intake assessments as required.

8) In an exit interview on 2/01/22, AR-2 acknowledged that the information was not contained in the Intake Assessments as required.Plan of Correction: The Foster Care Director, in collaboration with Quality Assurance will review the biopsychosocial template to make certain that all required elements are listed and upon completion that all required elements have been addressed.
