Comments:
Type of inspection: Renewal
A monitoring inspection was initiated and conducted on 6-12-24. The Director of Quality Assurance was available during the duration of the inspection as needed.
The exit meeting was conducted 6-26-24.

Number of youth records reviewed: 2
Number of staff records reviewed: 2

Number of interviews conducted with staff: 2

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.
If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.
Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
The department's inspection findings are subject to public disclosure.

Standard #: 22VAC40-191-40-D-1-e
Description: Based on record review and correspondence with the agency, the agency failed to obtain the last central registry and criminal history record checks before three years since the dates of the last background checks for Provider Home (PH-2).

Findings:
1) In review of PH-2; foster parent (FP1) & (FP2) both had a prior central registry (CR) finding dated for 5-10-2021 and a most recent CR dated for 5-21-2024, which was observed to be after 3 years since the dates of the last central registry finding. (3yrs 11 days).

2) In review of PH-2; FP1 had a prior criminal history record check dated for 4-9-2021 and a most recent criminal history record check dated 4-18-2024, which was observed to be after 3 years since the dates of the last criminal background checks. (3yrs. 9 days). FP2 had a prior criminal history record check dated for 4-12-2021 and a most recent criminal history record check dated for 4-18-2024, which was observed to be after 3 years since the last criminal background check; (3yrs. 6 days).

3) As of June 13, 2024, during the preliminary findings review, S3 acknowledged the findings.

Description: Violation:
Based on record review and correspondence with the agency, the agency failed to obtain the required criminal history record dated no more 90 days prior to the date of approval for PH-4

Findings:
1) The criminal history on file for review for PH-4 (approval date 2-7-2024) was dated more than 90 days prior to the date of approval, with a criminal history date of 11-8-2023 (91 days).

2) As of June 13, 2024, during the preliminary findings review, S3 acknowledged the findings.
