Comments:

A complaint inspection was conducted due to a complaint allegation of physical abuse and due to a report related to other household members. Due to the outbreak of COVID-19, the inspection was conducted in the form of a desk review on the following dates: 04/23/2020, 04/24/2020, and 04/27/2020. During this inspection portions of one (1) provider home record was reviewed and portions of (1) child record was reviewed and an agency representative was interviewed was conducted. An exit interview was conducted by video conferencing on 05/04/2020 at 3:00 pm with the State Foster Care Director, VA Beach Director of Therapeutic Foster Care, VA Beach Foster Care Supervisor and State Administrative Assistant. The complaint is deemed as invalid. There was (1) violation determined during this inspection. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days.

Description:	Based on an interview with Agency Representative (AR) 1 and review of the foster home application, the licensee failed to review the application for completeness. Findings: 1.Based on the review of the foster home application for FH1, the licensee failed to review the foster home application for completeness as evidenced by blank and incomplete sections of the application. Based on the review of the foster home application, there were multiple sections left blank on the foster home application including but not limited to page 5 of 8, "Other household members residing in the home." 2. Based on an interview with AR1, held by phone on 05/01/2020 at 2:05 p.m., Bair's policy is to review the foster home applications and to amend the applications by reviewing the blank areas of the application with the provider and having the foster parent applicant initial the updated areas of the application. During the interview, AR1 acknowledged that there were blank areas on the application.
Plan of Correction:	All Foster Parent Applications will be reviewed for completeness. 5/15/2020 Intake Coordinator Supervisors will review all foster parent applications to ensure they have been completed. 5/15/2020 Program Director Program Supervisor
