63.2 (1) General Provisions

Technical Assistance:
Technical Assistance provided regarding standard 22VAC40-131-260-B.12 format for including information in the social history from previous placements.

Comments:
A monitoring inspection was initiated on July 23, 2021 and concluded on July 30, 2021. The Program Director was contacted by telephone to initiate the inspection. The Program Director reported that the current census was 40 foster children and 41 foster home providers. There were 4 newly hired staff since the last inspection. The inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 4 foster child records, 5 complete foster home provider records and 3 additional foster home records (background checks and suspension records only) submitted by the facility to ensure documentation was complete. An exit interview was conducted with agency representatives on July 30, 2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or laws, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-131-180-J-1
Description: Based on the review of the records submitted for Foster Home (FH)-4, the licensee failed to document driving records obtained from the Department of Motor Vehicles for the applicant on the home study.
Findings:
1.The section of the home study for DMV was left blank.
2.There was no information in the home study for FH-4 regarding the driving records from the Department of Motor Vehicles.

Standard #: 22VAC40-131-190-O-6
Description: Based on the review of the records submitted for Foster Home (FH) 1-5, the licensee failed to develop a written emergency preparedness and response plan that addresses procedures to ensure that the plan is reviewed with each placed child.
Findings:
1.The Emergency Preparedness plans for FH 1-5 did not address procedures to ensure that the plan is reviewed with each child.
2.During the exit interview agency representatives acknowledged that the emergency preparedness plans did not address procedures to ensure that the plan is reviewed with each placed child.
3.During the interview AR 3 acknowledged that the emergency preparedness and response plans do not address procedures for ensuring that the plan is reviewed with each placed child. However, AR3 stated that during quarterly reviews the provider reviews the plan with each child.

Description: Based on the review of the records submitted for Foster Home (FH) 1-5, the licensee failed to develop a written emergency preparedness and response plan that addresses plans to rehearse the plan with each child at least once time every six months.
1.The Emergency Preparedness plans for FH 1-5 did not address plans to rehearse the plan with each child at least once time every six months.
2.During the exit interview agency representatives acknowledged that the emergency preparedness plans did not address plans to rehearse the plan with each child at least once time every six months.

Description: Based on the review of the records submitted for review for Foster Child (FC)-1 and based on an interview with agency representatives, the licensee failed to complete a preplacement visit for the child in the prospective home.

Findings:
1.In a section, titled ?document details of the pre-placement visit,? on the document titled ?Pre for Placement? dated 04/27/2021, the following is documented, ?N/A-pre placement visit did not occur.?
2.The summary did not document as an emergency placement.
3.During the exit interview, AR1 and AR 5 acknowledged that the placement was not documented as an emergency placement and acknowledged that a pre-placement was not completed.

Standard #: 22VAC40-131-330-G-3
Description: Based on the review of the records submitted for review for Foster Child (FC) 1-4 and based on an interview with agency representatives (AR), the licensee failed to document on contact notes the training and guidance to foster parents.

1.During the exit interview, AR1, AR4 and AR 5 reported that contact notes are titled ?Foster CM log? and ?VA Visit Coaching Form.?
2.During the exit interview, the inspector presented ?Foster CM log? and ?VA Visit Coaching Forms? pointing out behavioral concerns and other areas of clinical need.
3.Neither the ?Foster CM Logs? nor the ?VA Visit Coaching? forms documented raining and guidance provided to the foster parent.
4.During the exit, AR2 and AR5 acknowledged the absence of the documentation of training and guidance provided on contacts.
