Type of inspection: Monitoring
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 10/3/2022 from 10:00a.m. and 10/4/2022 (9:30 a.m. to 4:00p.m., and 10/5/2022 from 9:30 to 2:00p.m. The first day of the inspection was canceled due to a hurricane advisory and shutdowns in the area.
The Licensing Inspector met with the Program Assistant and the Program Supervisor at the site to start the inspection.

Number of children?s records reviewed: (4). Number of provider home records reviewed: (6) provider homes.
Number of staff records reviewed: (3) new staff reviewed during this inspection. The licensing inspector conducted separate interviews with each of the new staff. Observations by licensing inspector: posting of the current license. An exit meeting was conducted on the last date of the inspection. There were no changes made to the policies or procedures since the last inspection.
The evidence gathered during the inspection determined (2) violations with applicable standard(s) or law.

Standard #: 22VAC40-131-290-B Description: Based on the review of the records presented for Foster Child (FC)-3 and based on an interview with Agency Representatives (AR), the licensee failed to arrange for a vision examination for the individual that was no more than 13 months.
Findings:
1. Based on the review of the health examination record for FC3-3, the vision examination record reviewed was dated for 2012.
2. During the exit interview AR-3 and AR-4 acknowledged the absence of documentation to support a vision exam.Plan of Correction: Not available online. Contact Inspector for more information.
