Date of inspection: 11/13/23; 10:30-5pm and 11/14/23; 9:30-5pm. Licensing inspectors present: Irvin Goode and Sherry Woodard. Licensing inspectors met with Program Director Jennifer Gillam and Program assistant LaVanjulena Hawkins.

Board of Directors were reviewed during this inspection.
Observations by licensing inspection: Posting of license was observed. Provider has hired three new staff since last visit. Case Manager hired 9/2023 was interviewed and was able to verbalize understanding of job responsibilities. A preliminary findings meeting was conducted on 11/14/23 with the program Director, Program Assistant, and the State Director of Foster Care. The acknowledgement form was provided. The inspection documents were sent to the licensee on 11/29/2023.
The evidence gathered during the inspection determined (4) violations with applicable standard(s) or law. Standard #: 22VAC40-131-290-C-5 Description: Violation: Based on the record review for FC1, the agency failed to demonstrate visual acuity was addressed on the medical examination report signed and dated on 3/21/23.
Findings:
1. Physical examination documentation dated 3/21/23 in the record for FC1 did not address visual acuity. The visual acuity section on the medical examination form stated ?not done? in the space provided.
2. The findings were discussed during the preliminary findings and exit review.Plan of Correction: Program Director will train staff on checking medical forms to make sure they are fully completed before being put in the file. Foster parents will be trained on what needs to be filled out on medical forms by the doctor
Standard #: 22VAC40-131-290-C-6 Description: Violation: Based on the record review for FC1, the agency failed to demonstrate that auditory acuity was addressed on the medical examination report signed and dated on 3/21/23.
Findings:
1. Physical examination documentation dated 3/21/23 in the record for FC1 did not address auditory acuity. The auditory acuity section on the medical examination form stated ?not done? in the space provided.
2. Standard #: 22VAC40-131-290-F Description: Violation: Based on the record review for FC2, the agency failed to include documentation from the medical specialist visit on 6/13/2023.
Findings:
1. The follow-up medical visit in the record dated 6/13/23 for FC2 did not include the medical documentation.
2. The findings were discussed during the preliminary findings and exit review.Plan of Correction: Program Director will train staff on documenting upcoming visits in the file reminding staff to schedule follow-up recommendations by the doctor.
Standard #: 22VAC40-191-60-C-1 Description: Violation: Based on record review for S1 hired 10/16/23, the agency was not able to demonstrate an original criminal history record report within 30 days of employment.
Findings:
1. At time of renewal inspection visit on 11/14/23, State police documentation in record for S1 was returned to the requestor on 10/20/23 due to incomplete documentation. Written correspondence reviewed in the record for S1 from State Police outlined additional information needed for processing. Agency/requestor resubmitted documentation to State Police on 10/26/23. Criminal history results are pending as of 11/14/23.
2. The findings were discussed during the preliminary findings and exit review.Plan of Correction: Program Assistant will ensure background checks are completed before sending them out
