Type of inspection: Monitoring Dates of inspection: 8/29/24; 12:35-3:30pm review of staff records only and on 9/4/24; 1-4:45pm. Licensing inspector met with Program Director and Program assistant. Number of youths in care: 18 Number of youth records reviewed: (2) and (1) discharge record Number of staff records reviewed: 3 Number of interviews conducted with staff: 2 Additional Comments/Discussion: Observations by licensing inspection: Posting of license was observed. Provider has hired two new staff since last visit. A preliminary findings meeting was conducted on 9/4/24 with the program Director and Program Assistant. Licensing inspector had discussion with agency representatives about received date on materials, updates to case records and criminal history checks for staff. The acknowledgement form was provided, exit meeting was conducted on 9/6/24 and inspection documentation was emailed to the licensee on 9/11/24. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary. Description:	Violation: Based on review of record for staff S (1), there was no documentation in the record to determine that a criminal history record check was completed. Findings: 1. Staff S (1) was hired by child placing agency on 7/24/2024. 2. Sworn statement documentation and central registry results were reviewed in record for Staff (S1). Record review for (S1), did not include results of criminal history record check s at time of record review on 8/29/24. Agency was not able to demonstrate criminal history record check for Staff (S1) was completed before 30 days of employment ended. 3. The findings were reviewed during the preliminary finding meeting and exit meeting. Staff S3 acknowledged the findings during the preliminary and exit meeting and provided explanation for the identified findings.
